The safety question people ask about biotin is what it does to the body. The answer is: not much, it is water-soluble and well tolerated.

The question worth asking is what it does to the blood test.

The number the assay was built around

FDA guidance is explicit about the assumption underneath a large share of routine laboratory testing:

devices using biotin/avidin technology have been assessed for biotin interference at the normal recommended daily doses of biotin (30 μg per day, which results in plasma/serum biotin levels of < 1 ng/mL)

Thirty micrograms. That is the intake the test was validated against.

The middle row is the ordinary case, not the extreme one, and it already clears the interference threshold UC Davis reports at 20 ng/mL. The bottom row reaches a thousand times the level the assay was checked at.

Why extra biotin breaks a test

Many immunoassays use the biotin-streptavidin bond as their capture mechanism: it is one of the strongest non-covalent bonds in biology, which is exactly why laboratories build on it.

Free biotin circulating in the patient’s sample competes with that chemistry. The assay is no longer measuring only what it was pointed at.

The direction of the error is not fixed. FDA:

Biotin levels in samples from patients who consume more than the recommended daily intake for biotin can cause falsely high or falsely low results, depending on the test principle.

Which means there is no rule of thumb you can apply to your own results at home.

Troponin, and why it is the one that matters

A falsely high result is inefficient. Somebody investigates something that was never there.

Troponin runs the other way. It is the marker used to decide whether chest pain is a heart attack, and on affected assays biotin pushes it down. A low troponin does not prompt a second look. It closes the question.

The FDA maintains a public list of troponin assays that have not addressed this. Current as of 21 June 2022, it names thirteen, including:

  • Ortho-Clinical Diagnostics — VITROS Troponin I ES

The agency’s stated reason for publishing names at all: it “has continued to receive adverse events reports indicating biotin interference caused falsely low troponin results.”

The death, described accurately

You will find this reported as biotin killed a patient. That is not what the record supports, and overstating it would make the rest of this page less trustworthy rather than more.

A patient taking high-dose biotin died after a potentially falsely low troponin result, and that case prompted the FDA warning. UC Davis Health’s laboratory review adds the two qualifiers that matter:

biotin interference was not confirmed and troponin measurements following the event were made post-mortem

One reported case, causation unproven, set against a very large number of people taking biotin. It was enough to move a regulator. It is not enough to frighten anyone.

Where the threshold actually sits

The FDA gives the validation assumption; the clinical laboratories give the working number. UC Davis reports that the interference threshold at which some assays shift by more than 10% has been reported at 20 ng/mL, and that in people taking up to 10 mg daily, serum biotin can reach around 30 ng/mL eight hours after a dose.

Ten milligrams is over three hundred times the 30 micrograms the assay assumed. A morning supplement and an afternoon blood draw are the collision.

What to actually do

Not stop. The FDA has not asked anyone to stop, and this page is not going to issue a stronger instruction than the agency did.

  1. Tell whoever draws your blood that you take biotin, and at what dose. This is the entire fix and it costs nothing.
  2. If a result does not match how you feel — a thyroid panel saying one thing while your body says another — name biotin specifically rather than wondering vaguely.
  3. Ask the ordering clinician about pausing before a planned draw. Not this page: the washout depends on your dose and on which assay their laboratory runs, and no honest general number exists for that.

And on whether it was worth taking anyway

That is a separate question, and the short version is that true biotin deficiency is rare and the evidence for supplementing people who are not deficient is thinner than the marketing suggests.

What changes here is the arithmetic. A documented deficiency makes the trade easy. Taking a high dose on spec for hair thickness now has to clear a second bar: whether it is worth carrying a measurable interference risk into every blood test you have while you take it.

The FDA has asked manufacturers to evaluate their assays up to 3,500 ng/mL, three times the maximum expected clinical concentration. That number is the real measure of how far consumption has moved from the 30 micrograms the tests were built around.

Which tests are affected?

Any immunoassay built on biotin-streptavidin capture, which is a large and unglamorous share of routine testing. FDA guidance names hormone tests and tests for markers of cardiac health like troponin. Thyroid tests are the ones patients most often notice, because the pattern biotin produces can mimic hyperthyroidism convincingly enough to prompt treatment. The direction of the error is not fixed: FDA states that biotin levels in samples from patients who consume more than the recommended daily intake can cause falsely high or falsely low results, depending on the test principle. So this is a reported case that prompted regulatory action, not a proven causal chain. It is also a single case set against a very large number of people taking biotin, which is worth holding in mind alongside it.

Should I stop taking biotin?

That is not what the FDA asks for, and this page is not going to invent a stronger instruction than the agency issued. The problem is a measurement problem, and the fix is disclosure: tell your doctor, and tell the person drawing your blood, that you take biotin and at what dose. If a test result does not fit the clinical picture — a thyroid panel that says one thing while you feel another — biotin is a specific thing to raise rather than a vague worry. Whether to pause it before a planned blood draw is a question for the clinician ordering the test, because the washout depends on your dose and on which assay their lab runs.

Is this a reason not to take biotin for hair?

How long does biotin stay in the blood?

Long enough to matter for a same-day test and not much longer, but the specifics vary and this page will not invent a number the sources do not give. Biotin is water-soluble and cleared renally, and UC Davis's figures describe levels measured around eight hours after ingestion, which is the window where a morning gummy and an afternoon blood draw collide. What the published sources here do not establish is a general washout period that is safe to quote for every dose and every assay. Ask the ordering clinician; they can check what their laboratory's assay tolerates.
